DATA SUBJECT RIGHT REQUEST FORM
As Doğadan Gıda Ürünleri Sanayi ve Pazarlama A.Ş. (“Doğadan”), the protection of fundamental rights and freedoms, particularly privacy of private and family life, ensuring information security and respecting ethical values are our primary concern. Article 11 of the Law on the Protection of Personal Data numbered 6698 (“LPPD”) titled “Rights of the Data Subjects” provides certain rights for data subjects in connection with the processing of their Personal Data. As the Data Controller, Doğadan provides this application form in order data subjects to exercise their rights and fulfill its obligation to provide privacy notice under article 10 of LPPD.
Pursuant to article 11 of LPPD, you may exercise the rights listed below by applying Doğadan and filing this application form:
a) Obtain confirmation as to whether or not personal data concerning you are being processed,
b) Where personal data is being processed, obtain information about processing activities
c) Obtain information about the purposes of the processing activities and whether your personal data they are used in accordance with relevant purposes,
d) Obtain information on the recipients to whom the personal data have been disclosed either in recipient in Turkey or in third countries,
e) Right to obtain the rectification of inaccurate personal data concerning you,
f) Right to obtain the erasure of personal data concerning you where grounds stated in Article 7 of LPPD applies,
g) Right to notification of procedures implemented as per paragraphs (e) and (f) above to the recipients of Personal Data,
h) Raising an objection against the results detrimental to you due to analysis of your Personal Data exclusively by automated means,
i) Claiming remedy for the losses incurred due to unlawful processing of your Personal Data in accordance with LPPD.
Pursuant to article 13 of LPPD, the data subjects should forward their demands in writing in the applications to be submitted to our Company or with the use of other methods to be determined by the Turkish Data Protection Authority (“Authority”). Applications to be submitted to Doğadan may be forwarded to us without any charge by choosing one the four methods indicated below after completing this form and taking a printout of it.

Third persons are not allowed to exercise rights of obtaining information under article 11 of LPPD on behalf of the data subjects. If the data subject wishes to make demand in connection with the Personal Data of a third person, he/she must submit a wet signed and notary public attested power of attorney conferring rights on the third person to apply on behalf of that data owner.
If the demand made by the relevant person requires payment of extra costs, our Company will charge the fees indicated on the tariff determined by the Authority.
Your applications forwarded to us will be finalized within thirty days after it is received by Doağadan depending on the nature of demand as per the provisions of article 13/2 of LPPD. Pursuant to article 13 of LPPD, our responses will be forwarded to you in writing or by electronic means.
